The Bedford-Stuyvesant Family Health Center (BSFHC) is a Federally Qualified Health Center (FQHC) that serves all of the primary health care needs of families in the heart of North and Central Brooklyn for over 40 years. Our mission is to provide the most professional, courteous and highest quality health care, with dignity, to those we serve, especially the undeserved population, without regard for ability to pay.
We are seeking a Behavioral Health Care Manager to join our team. Behavioral Health Care Manager is part of the team concept in the Center’s health delivery system, which is devoted to the delivery of integrated behavioral health and primary care in an ambulatory setting with emphasis on prevention and health maintenance. The Behavioral Health Care Manager assists the Nurse Practitioner, Licensed Clinical Social Workers (LCSW), and other clinical staff in performing his/her duties and carrying on the necessary functions required in accordance with established BSFHC policies and procedures. This position involves working closely with the Behavior Health Team implementation strategies and aimed at encouraging patient retention and reaching out to past patients to encourage re-attendance at our center and any policies thereof to achieve BSFHC’s mission.
