Bear Mountain Healthcare is Hiring an Assessment Coordinator (formerly MMQ) Near Springfield, MA

We are seeking your skills and compassion as an MMQ Nurses/ Assessment Coordinator to come join our team!

Primary Responsibilities:

Assessment Coordinators generate UDA’s Prior to MDS ARD (with specific emphasis on GG and monitor for timely completion)

  • Ensure all UDA’s (nursing assessments) are filled out and locked
  • Ensure all UDA’s are done timely
  • UDA’s to be completed:
  • BIMS (Brief Assessment for Mental Status, interview typically done by Social Services). It is to be open within 7 days leading up to MDS ARD
  • PHQ-2-9 (Mood Interview, interview typically done by Social Services) to be opened within seven days leading up to ARD.
  • Pain Interview- typically done either by Nursing, MDS, or Social Services. To be opened within 5 days leading up to ARD
  • Pain assessment- typically done by nursing staff
  • Norton- typically done by nursing staff
  • Fall Assessment- typically done by nursing staff
  • Oral Assessment- typically done by nursing staff
  • GG, also called “Functional Abilities and Goals”

This is done by nursing staff in most facilities. GG assessment is used for reimbursement. If we don’t have it, we pay claims back for all payers.

  • OBRA UDA’s to populate the MDS’s are generated for admission, quarterly, annually, for Medicare 5 day, and for significant change MDS’s
  • Ensure care plans are present for all diagnoses for each pt. with an OBRA assessment for each resident (when OBRA’s are due)
  • The upcoming MDS’s are open in the “in progress list” in PCC to refer to.
  • Diagnoses must be present in Medical Diagnosis tab in PCC to support indication for medications the pt. is on
  • Ensure order for elevating HOB is present on the MAR or TAR (you may be the one to set up the order if it’s not there).
  • Ensure there is a respiratory care plan and that it has “elevate head of bed to prevent shortness of breath as tolerated” as an intervention
  • Assist with ensuring documentation is present for behavior add Ons
  • Perform MDS assessments as delegated to support MDS as needed
  • Work with your MDS Coordinator to ensure the MDS’s have the supporting documentation for what is coded on them (like MMQ supporting doc).
  • Cover for each other (MDS and Assessment Coordinator) in the event of vacations/ day off and make corporate support aware.
  • Communicate with Administrative and Nursing leadership if UDA’s are not being filled out and missing. May be required to perform assessments if nursing cannot complete as ultimately, they have to be done.
  • Daily monitoring of compliance with the current/ upcoming UDA’s by the Assessment Coordinators is paramount.

  • Morning clinical meeting
  • Medicare Meeting
  • At Risk Meeting/Behavior Meeting
  • QAPI

Minimum Job Skills Requirements

  • Must hold a current RN or LPN license in MA
  • MMQ experience preferred
  • Must be covid vaccinated per MA regulations
