We aim to make medical care more comfortable and convenient for patients by providing comprehensive services in the security of their own home. We are looking for a LPN / Certified Medical Assistant to work with our team of Nurse Practitioners. Daily duties include reviewing clinical orders, communicating with patients and providers. Required skills are great phone presence, precision, analytical problem-solving abilities, excellent communication skills, focus and initiative. Previous medical office and EHR experience a plus. This is an in office / administrative position.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

• Responsible for providing support to clinicians through patient triage, documentation, providing appropriate levels of understanding, maintaining and managing patient forms and referrals.
• Maintain open communication with management, clinicians, and nursing staff.
• Collaborate with multidisciplinary team to assist with problem identification, resolutions & implementation of systems.
• Maintain contact with patients, clinicians & other members of the healthcare team in a timely manner regarding patient/family needs and status changes.

QUALIFICATIONS:

• Current Florida LPN License/ Certified Clinical Medical Assistant required
• Two (2) years of medical experience required.
• Primary care/geriatrics experience a plus.
