Overview:
We are seeking a dedicated Social Service Assistant to join our nursing home team. The ideal candidate will have experience in grievance processes, PASSR, discharge planning and other administrative duties. If you have a background in behavioral health and social work, we encourage you to apply.
Duties:
- Manage and coordinate services for individuals in long term care facilities
- Develop and implement individual service plans
- Supervise and support residents needs
- Conduct assessments to determine resident needs
- Collaborate with community resources and agencies
- Monitor program outcomes and make recommendations for improvements
Experience:
The successful candidate should possess the following skills:
- Administrative expertise
- Strong relationship management skills
- Experience in behavioral health or social work
- Working knowledge of developmental disabilities
- Budgeting skills for program management
- Ability to develop and manage programs effectively
- Experience in supervising staff
