We are seeking registered nurses (RNs) to conduct home care assessments and UAS assessments. The RN Home Care Assessor Nurse will be responsible for completing assessments in patients' homes, completing tasks and tools related to the assessments, and ensuring that all necessary paperwork and electronic documentation are properly completed and submitted.

• Full-Time Salary: $120,000

Fee For Service Rates:

• Start of Care: $120
• UAS Re-assessments: $120
• Home Care Re-assessments: $100
• Aide supervision Visits: $80

Responsibilities:

• Performing visits in the home setting to conduct functional, psychosocial, and environmental safety assessments
• Gathers clinical information from various sources to address all conditions, and follows appropriate guidelines, policy, procedure, and clinical judgment in completing assessment requirements.
• Conduct needs assessment by utilizing UAS-NY/DOH tool to determine consumers’ qualifications and appropriateness for Long Term Care Services.
• Create service plans and determine eligibility and continuing program services via the UAS.
• Ensuring that DOH UAS information is complete and accurate
• Conduct follow-up assessments to ensure members receive the proper care and service

Job Types: Full-time, PRN, Permanent, Per diem

Pay: $70.00 - $90.00 per hour
