Position Overview:

The RN for the Home-Based Primary Care programs holds the responsibility for implementation of the Vision, Mission and Values of the Bay Pines VA Healthcare System. He/she is engaged in administrative nursing practice characterized by both increased complexity of clinical decision making and enhanced skill in nursing management. The RN is responsible for providing care to home bound Veterans who have complex health care need for whom routine clinic-based care is not effective. The Home-Based Primary Care Team is an interdisciplinary team that works collaboratively to provide intensive case management longitudinally in coordination with other community-based programs. This position will require travel up to 75%. BSN preferred.

Tour of Duty: Day Tour – 730am – 4pm with up to 75% travel.

Qualifications:

The Staff RN/Case Manager is a US Citizen and is currently licensed as a Registered Nurse with an active, current, full, and unrestricted license. The incumbent is a graduate of a state approved school of nursing and accredited by either the Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). Bachelor’s degree in nursing or related field is preferred with five years of successful nursing practice in Med-Surg, ER, Hospice, and Home Care. Verbal and written communication skills are required. Maintaining a current BLS certification is required. The RN Case Manager has the physical and mental capability to satisfactorily perform assigned duties.

Duties:

The RN Case Manager (CM) provides case management and nursing care for veterans with complex care needs, assuming responsibility and accountability for nursing actions for a case load of 25-30 patients residing in their own homes or Adult Congregate Living Facilities. Completes initial assessments and reassessments of client condition and response to treatment and other clinical findings. Initiates follow-up as needed. Works collaboratively with interdisciplinary team members working closely with Primary Care Providers and the multidisciplinary team. The RN CM acts as a patient advocate, initiates action to resolve ethical/legal issues related to nursing practice in collaboration with appropriate resource people, identifies potential problems involving resources and/or safety and takes action to avert or manage the situation in a manner which meets the needs of clients in a safe/therapeutic manner.

The RN Case Manager frequently engages co-workers/team in active problem-solving regarding clinical issues leading to an improvement in care, collaborates with other disciplines and caregivers and uses group process to identify, analyze, and resolve problems affecting care. The RN works effectively with others by resolving conflicts and assisting others to cope with stressful situations. The RN Case Manager serves as a resource to other staff in meeting individualized learning and discharge needs, recommends appropriate referrals, adaptive equipment, and approved educational materials. Documents data related to assessments, care provided and outcomes in the medical record.

Pay: $68,599.00 - $133,628.00 per year
