Job Summary
Deliver in-home care services to Bay Agingclients so they can live safely in the community. Provide personal care, healthand safety monitoring and light housekeeping. Introduce and coach new PersonalCare Aides in the field. Perform administrative work in the office to completea full-time schedule every week. Report to the Registered Nurse (RN) Supervisor.
Essential functions
·Follow the individual Care Plan developed by the RNSupervisor.
1)Assist with Activities of Daily Living (ADL)
-Bathing,dressing and grooming
-Toileting
-Transferring
-Assistwith eating
-Ambulation
-Turnand change position
2)Provide assistance with Specialmaintenance
-Checkvitals,
-Supervisemedication
-Assistwith specialized care need as assigned by the RN supervisor
-Providesupervisory care for clients who require constant monitoring.
3)Perform IndependentActivities of Daily Living (IADL)
-Mealpreparation.
-Lighthousekeeping in the area of the home used by the client.
-Laundry.
-Accompanyclient to medical appointments, post-secondary school or in the communitysetting when assigned.
·Provide assistance with errands outside the home. Run errands within thelocal area for the client (e.g. grocery store, pharmacy, Post Office)
·Monitor home environment for health and safety.
·Communication and training.
-Documentall services provided. Complete Aide records each day. Submit Aide records asdirected.
-Communicateall changes in client’s physical or mental condition to RN Supervisor includingsuspicions of exploitation, abuse or neglect.
-Completeat least 12 hours of formal training annually.
-Provideon-the job training and coaching to new PCAs.
·Office work (up to 5%)– Workunder the direction of the RN Supervisor or Scheduling Coordinator, to perform office administrative duties including but notlimited to filing charts and making copies.
DECISIONMAKING
Perform a limited number of routinetasks prescribed by clients’ care plan. Work independently throughout the day.Report to supervisor any change in the client’s environment, physical or mentalcondition.
KNOWLEDGE
Working knowledge of:
-HIPAAregulations.
-Adultcare practice and procedures.
-BayAging policies and procedures related to Home Care.
Has knowledge of medical equipment on usage andbasic maintenance such as Hoyer lift, gait belt, wheelchairs, oxygen tanks,catheters, glucometer and any specialized equipment required by the care plan.
Stay current in the knowledge of theirprofession by completing at least 12 hours of in-service annually
ABILITIES
-Speak,read and write English
-Establishand maintain good relationships with clients and their families even indifficult circumstances.
-Communicateeffectively with older adults suffering from dementia or Alzheimer’s disease. Communicatewith clients’ families to ensure good continuity of care.
-Beconsiderate of care recipients and hold their personal information as confidential.
-Beobservant of changes in client’s condition or environment.
-Abilityto stand, bend, stoop, pull, push wheelchairs and lift up to 25lbs unassisted.
WORKINGENVIRONMENT
The work is performed in the client’s home andin public areas. It requires repeated standing, bending, stooping, pulling,pushing and lifting of weights up to 25lbs unassisted. Regular night, weekendand holiday work required on a regular basis. None of the essential functions of the positioncan be performed virtually away from the assigned places of work.
QUALIFICATIONS
-Mustbe 18 years of age
-Hold a Certificate of Completion for a Personal Care Aide (PCA)course that meets the Department of Medical Assistance approved curriculum.
-Must provide proof of current PPD vaccination or complete aTuberculosis screening.
-Become certified in CPR.
