Salary: Starting at $50,000

General Summary of Duties: We’re looking to add a Licensed Clinical Social Worker to our growing team of medical professionals. Your primary responsibility will be making clinical decisions, providing counseling with a focus on brief therapy and community transitions. You should have knowledge of crisis services, mental health, and long-term care resources. Duties will include but are not limited to assisting in healthcare referrals, expertise in group work like cognitive behavior therapy, and active participation in data management for quality improvement. The role involves effective communication of measurement results, patient advocacy, liaison with healthcare providers, and teaching family medicine residents.

A Master’s degree and 2 years related experience is preferred for you to be considered as a candidate. Experience in solution focused brief therapy, cognitive behavior therapy, motivational interviewing, and mindfulness are important keys to becoming successful in this role.

Typical Working Conditions: Multiple site urban, inner-city medical office practice. Requires prolonged standing, sitting, bending, stooping, and lifting.

Duties and Responsibilities:

  • Is responsible for making clinical decisions, exercising judgement to ensure best possible care is provided within available resources.
  • Provides counseling services regarding individual and family issues and decisions that impact the health of the patient and family. Works within primary care model of brief therapy, ensuring seamless transition to community resources as appropriate.
  • Is knowledgeable of the community and the resources available, especially in the areas of crisis services, mental health, and long-term care.
  • Assists the referring physician/health care providers with referrals to secondary and tertiary health care centers.
  • Knowledge and skill in group work including cognitive behavior therapy and mind over mood.
  • Enthusiastic, passionate, and self-driven who will participate in data management and measurement of quality improvement indicators of patient outcomes.
  • Communicates results of measurements that are easy to use and understand, looking for patterns that could assist with improvements.
  • Acts as advocate for patients and their families in matters affecting health.
  • Liaises with physicians and other team members as required.
  • Teaches family medicine residents and other health care professional students on the role of the social worker and psycho-social issues facing patients and families.
  • Effectively uses the social work standards of practice and the therapy process in assessing, planning, implementing, and evaluating the care of the patient.
  • Maintains own continuing education to enhance professional knowledge and growth.
  • Participates and collaborates with other family health team members and health care providers in the provision of patient care.
  • Acts as a professional and knowledgeable role model or mentor to colleagues and students.
  • Maintains confidentiality in the workplace within the community.

  • Fully participates and cooperates with BRPCC compliance program.
  • Meets dress code standards; appearance is neat and clean.
  • Completes annual educational requirements.
  • Maintains regulatory requirements.
  • Wears identification while on duty.
  • Maintains employee confidentiality.
  • Communicates the mission, ethics, and goals of the facility as well as the focus statement of the department.

Supervisor: Reports directly to Clinical Director.

Education/Experience:

  • Master’s Degree preferred.
  • Minimum 2 years’ experience preferred.
  • Registered with the Louisiana State Board of Social Work Examiners
  • Experience in solution focused brief therapy, cognitive behavior therapy, motivational interviewing, and mindfulness.
  • Previous related patient care experience, specifically in a primary care setting preferred.
  • In-depth knowledge of chronic illness care, including both clinical and supportive care.
  • In depth knowledge of the local health program delivery and community-based services
  • Ability to be innovative and creative in order to develop strategies that successfully meet the needs of diverse patients, family, and provider population.
  • Strong conflict resolution
  • Exceptional oral, written, and presentation skills.
  • Strong interpersonal skills
  • Group facilitation skills

Working Conditions: Work is performed in an office environment.

  • Able to move intermittently throughout the workday.
  • Able to sit and/or stand for long periods of time.
  • Ability to prioritize, manage time effectively and be flexible in a very active work environment.
  • Possesses sight/hearing senses or uses prosthetics that will enable these senses to function adequately so that the requirements of this position can be fully met.
  • Possesses flexibility, personal integrity, and the ability to work effectively with other staff members.
  • Able to operate business machines, i.e., computers, copy machine, fax machine.

Compensation: Starting at $50,000 (Salary range commensurate with education and experience.)
