Barber National Institute is Hiring an Agency With Choice - AWC Family Liaison Near Erie, PA
The Family Liaison is responsible to coordinate and maintain effective working relationships with the families and staff in the Agency With Choice (AWC) program as well as oversight of vendor services for the Organized Health Care Delivery System (OHCDS). This includes maintaining a working knowledge of requirements, regulations, and funding for the program. The AWC Family Liaison will serve as the AWC and OHCDS contact for a designated caseload of individuals/representatives. This role requires extensive time management and organization skills. Applicants should be detail oriented and confident in their ability to consistently provide quality service to both families and program staff. AWC Responsibilities:
Oversight of designated caseload of around 40-45 individuals
Managing annual budgets of individuals on designated caseload
Reviewing and approving Support Service Professionals time cards
Completing data entry for the payroll process
Knowledgeable about person-centered planning; the guiding principles of self-determination and the minimum guidelines for choice and control
Report any mandatory reportable incidents that take place during provision of services
Support families in recruiting support service workers
Discipline and terminate support service workers as appropriate
Ensure workers complete documentation as required
Complete invoicing for services rendered
Generate standard reports to the Individual/Representative as required, including but not limited to quarterly financial statements.
Assure all adaptive equipment and home adaptations purchased are utilized for the benefit of the individual.
Qualifications:
Bachelor's degree required
2 years’ experience working with individuals with developmental disabilities preferred
Conflict management experience preferred
Must have a valid driver’s license and good driving record
Eligibility for State Police Criminal Record Check and Child Abuse History Clearance
