Baptist Health Nursing and Rehabilitation Center has a full time opening in the Social Work Department as a Rehab Social Worker.

Job Requirements:

Education –

1. BSW required, MSW preferred

Experience –

1. Previous experience in transition/sub-acute rehabilitation care preferred.

2. Two or more years of experience working in a residential facility.

3. Experience in admission and discharge planning and care coordination.

Skills –

1. Ability to provide case coordination, admission and discharge planning with an awareness of available community services.

2. Ability to work as part of an interdisciplinary care team and work independently.

3. Knowledge of the MDS, depression and anxiety tools desirable

Essential/Primary Functions of the Job:

1. Provide Admission and Discharge services to the residents and families on our rehabilitation unit

2. Complete admissions paperwork, MDS documentation and plan safe discharges for residents

3. Advocate for the resident

4. Assist the resident with social and economic barriers to discharge

5. Develop care plans

6. Act as a liaison between resident and community service providers.

Computer, fax machine, copy machine, telephone, printer.

Job Type: Full-time

Pay: $58,000.00 - $65,000.00 per year
