Banana Kelly is a non-profit community development corporation located in the South Bronx. Our mission is to contribute to a revitalized, safe and economically vibrant South Bronx through the development and management of affordable housing and the provision of targeted human services. We empower residents through supportive services and vocational, educational, and cultural programs that assist in the retention of affordable housing accommodations and foster self-sufficiency and collective capacity through self-help, mutual aid and cooperation, community organizing and advocacy.
Banana Kelly is looking for a Case Coordinator to provide services to families/individuals who are formerly homeless. The successful candidate will be accountable for the light touch individual care cycle. Case coordinator duties include in-take, assessing, planning, linking, implementing, monitoring and evaluating actions required to meet the individual/family’s health and human services needed.
Requisites
· Minimum of a Bachelor’s degree in Psychology, Social Work or behavioral science related to the field from an accredited college or university.
· Minimum of 1,000 hours of supervised work experience in the delivery of light touch services to individuals/families who are formerly homeless.
· Must be licensed in the related field of practice.
Responsibilities
· Coordinate and provide case coordination care that is safe, timely, effective, efficient, equitable, and individual-centered
· Conduct comprehensive face-to-face assessment (virtual when COVID conditions necessitate) using Banana Kelly’s clinically appropriate assessment tool
· Complete all required assessment activities in a timely fashion
· Develop and implement a comprehensive ISP, Individual Service Plan, in conjunction with the individual and his/her family members as well as any existing social service providers
· Work collaboratively with the individual’s existing service providers
· Handle case assignments, draft service plans, review case progress and determine case closure
· Establish connections and/or referrals for services and monitor those connections for effectiveness
· Serve as primary contact for the individual
· Facilitate multiple care aspects (case coordination, information sharing, etc)
· Help residents make informed decisions by acting as their advocate regarding their clinical status and treatment options
· Develop effective working relations and cooperate with existing social service providers throughout the entire case coordination/management process
· Interact with individuals and family members to keep track of their progress and to ensure satisfaction
· Record cases information, complete accurately all necessary forms and produce statistical reports
· Assess and address motivational and psychosocial issues
· Adhere to professional standards as outlined by industry standard protocols, rules and regulations
· Remain knowledgeable and up to date with case coordination requirements provided by NYC DSS/HRA
· Participate actively in on-going professional growth and development; maintain appropriate professional behavior and participate in appropriate supervision
Requirements
· Proven working experience in case coordination/management, including as a nurse, medical, mental health care manager or a related job
Spanish speaker a plus
· Excellent knowledge of case management principles
· Knowledge of community resources
· Previous experience with psychological aspects of care
· Effective communication skills; must be organized and able to attend to detailed paperwork
· Excellent organizational and time management skills
· Familiar with or able to learn data base management
· Able to work independently and as a member of an interdisciplinary team
· Familiarity with professional and technical emerging knowledge
· Problem solving skills and ability to multi-task
· Compassionate with teamwork skills
· Current license, certification or registration
· BS degree in related healthcare field
