Baltimore Medical System is Hiring a Panel Manager Near Baltimore, MD

Under supervision of the Director of Population Health or designee, collaborate with a multidisciplinary team to improve health outcomes of PPMCO patients through population health and panel management strategies, with a focus on newly assigned patients during active phases of network optimization and on panel maintenance to meet value-based purchasing benchmarks. Panel Manager is responsible for providing Care Coordinators with information required to manage the care of PPMCO members. Using the 360 Report and the tools within the Report, Panel Managers are responsible for reviewing member demographic and health care utilization data to identify and refer members with outstanding health care gaps to the Care Coordinators with a particular emphasis on high need members defined as those with diagnoses of diabetes, hypertension or other cardiovascular diseases, HIV, asthma, sickle cell anemia, as well as members with open preventive care gaps. Panel Managers are also responsible for ensuring member demographic, enrollment, provider assignment and clinical data is accurate.

Primary responsibilities of this position include assisting patients in accessing all levels of care, assisting in the improvement of the quality of preventive and chronic care services, assisting the medical team in preventive care and chronic disease management, managing health information data, and participating in continuous quality improvement activities. This position is responsible for implementation, monitoring, and evaluation of novel patient outreach and engagement strategies considering barriers to care engagement, including social determinants of health. Shares information necessary to generalize best practices to all panels. Performs related duties as required.
