Shift: Day, Monday - Friday
Job Description: Provides general clinical and administrative oversight of organizational case management services and activities. Manages all aspects of community case management services, inclusive of capacity, sustainability, compliance, and quality assurance. Engages internal and external partners to support and enhance system integration activities. Contributes to the strategic plan for delivery of services within system(s) of care for individuals, families, and support systems. Works collaboratively with senior leadership team.
Essential Functions:
Education: Masters in Clinical Psychology, Social Work, Nursing, or related field from an accredited University or College, Required
Experience: 5 years as a care/case management professional, Required.
5 years of managerial or director level experience in managing human service programs, Required. 10 years Preferred.
5 years of experience working with clients across the life span including clear and notable experience with children, youth, and adults.
3 years of experience working in Baltimore City, Preferred.
Credentials: Licensed and legally authorized to practice under the Health Occupations Article, Annotated Code of Maryland in Social Work, Professional Counseling, Psychology, Nursing, Occupational Therapy, or Medicine, Required LCSW-C and/or LPC-C, Preferred, licensure supervisory credential preferred.
Additional: Valid and unrestricted driver’s license, Required
Proficiency with Microsoft Office Suite Products, Required
Proficiency and familiarity with EHR/EMR systems, Required Current BLS Certification, Preferred
Physical Demands: Prolonged periods of sitting at a desk or computer workstation.
Prolonged periods of standing, bending, and reaching. Must be able to lift weights up to 50 pounds at times.
Psycho-Social Demands: Seeks guidance / help proactively to support safety and quality care.
Demonstrates skill of distress-tolerance to maintain calm and safety.
Models positive regard with all members of the community of care.
