Baltimore Crisis Response, Inc is Hiring a Director of Case Management Near Baltimore, MD

Shift: Day, Monday - Friday

Job Description: Provides general clinical and administrative oversight of organizational case management services and activities. Manages all aspects of community case management services, inclusive of capacity, sustainability, compliance, and quality assurance. Engages internal and external partners to support and enhance system integration activities. Contributes to the strategic plan for delivery of services within system(s) of care for individuals, families, and support systems. Works collaboratively with senior leadership team.

Essential Functions:

  • Oversees the planning, implementation, and system integration of case management and care coordination services for children, adolescents, and adults.
  • Coordinates with internal and external stakeholders (including 988 Call Centers, Law Enforcement, Residential Services, MRT/MCT, Medical Services, Health Department, etc.) and participates in the planning, implementation, management, monitoring, and evaluation of case management services, and ensures all contractual deliverables with the local behavioral health authority are met timely and adequately.
  • Oversees the development of organizational strategic plan for case management and care coordination programs, workflows, policies and procedures.
  • Responsible for designing and implementing case management and care coordination services according to contractual deliverables with LBHA.
  • Collaborates with senior management to integrate care coordination and case management needs in organization-wide initiatives,
  • including, not limited to, electronic medical records, revenue management, service-delivery, and business intelligence.
  • Oversees data collection, analysis, and auditing, and the preparation of presentations, proposals and executive summaries. Utilizes data and metrics to develop and prioritize quality initiatives and long and short-term strategic planning for programming.
  • Manage compliance with accreditation and licensure requirements.
  • Responsible for employee performance reviews, recruitment, retention, terminations, and staff development and training.
  • Monitors and tracks clinical documentation, service trends, and makes recommendations based on evidence-based best practices regarding service improvements.
  • Demonstrate adherence to company policies, expectations, and behavioral competencies, both explicit and implicit.
  • All tasks, projects, and/or duties as assigned and authorized, which are congruent as determined by Direct Supervisor, Department Head, and Executive Director.

Education: Masters in Clinical Psychology, Social Work, Nursing, or related field from an accredited University or College, Required

Experience: 5 years as a care/case management professional, Required.

5 years of managerial or director level experience in managing human service programs, Required. 10 years Preferred.

5 years of experience working with clients across the life span including clear and notable experience with children, youth, and adults.

3 years of experience working in Baltimore City, Preferred.

Credentials: Licensed and legally authorized to practice under the Health Occupations Article, Annotated Code of Maryland in Social Work, Professional Counseling, Psychology, Nursing, Occupational Therapy, or Medicine, Required LCSW-C and/or LPC-C, Preferred, licensure supervisory credential preferred.

Additional: Valid and unrestricted driver’s license, Required

Proficiency with Microsoft Office Suite Products, Required

Proficiency and familiarity with EHR/EMR systems, Required Current BLS Certification, Preferred

Physical Demands: Prolonged periods of sitting at a desk or computer workstation.

Prolonged periods of standing, bending, and reaching. Must be able to lift weights up to 50 pounds at times.

Demonstrates skill of distress-tolerance to maintain calm and safety.
