Azorna Healthcare is Hiring a LPN Hospice Nurse Near Mesa, AZ

We are looking for a qualified and compassionate LPN to help with our ever-expanding census in the East Valley. JOB SUMMARY:

The Licensed Practical Nurse is responsible for providing direct patient care under the supervision of a Registered Nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help care for hospice patients.

QUALIFICATIONS:

  • Educational/Degree: Graduate from an accredited practical nurse or vocational nursing program.
  • Training/Licensure: Currently licensed as an LPN to practice in the state of Arizona.
  • Knowledge/Skills/Ability: Ability to work independently, make accurate, and at times, quick judgments. Ability to supervise others appropriately. Ability to respond appropriately to crisis outside of a hospital setting. Acceptance of and adaptability to different social, racial, cultural and religious modes. Completes Hospice training program.
  • Must have a current valid driver’s license, auto liability insurance and reliable transport

Experience: Minimum 2 years of experience as a nurse, preferred.

Active patient contact within past three years, preferred

Hospice experience, preferred

Essential Functions:

  • Provides direct patient care as defined in State Nurse Practice Act.
  • Implements current nursing practice following a patient assessment and an approved plan of care initiated by the Registered Nurse.
  • Provide accurate and timely documentation of patient services to reflect the plan of care.
  • Teaches, supervises and counsels the Hospice patient and family members about providing care for the patient.
  • Observes signs and symptoms and reports to the physician and IDG members any unexpected changes in the patient’s physical or emotional condition.
  • Participates in on-call system and is responsible for providing on-call coverage when unavailable for assigned duties.
  • Performs other duties as assigned by the Case Manager and/or Supervisor.

Please note: Applicant will be required to pass a drug screening before beginning employment.

Job Type: Full-time

Pay: $28.00 - $32.00 per hour

Expected hours: 40 per week
