AZ CENTER FOR CHANGE is Hiring an Engagement specialist-Behavioral Health Near Phoenix, AZ

Job Summary:

AZCFC is a proven leader in the delivery of specialized, evidence based behavioral health services. We are seeking to expand our amazing referral and outreach team by adding Engagement Specialists. The Engagement Specialist provides focused support to individuals who may be experiencing barriers to enrolling in or following through with services. The Engagement Specialist provides a focused customer service approach by sharing their experience, strength, and hope to individuals, families who may have missed ongoing sessions and are having difficulty re-engaging. The Engagement Specialist provides support to staff and is able to assist with intakes when needed.

Essential Functions:

  • Assists in the coordination of activities associated with incoming referrals and ongoing coordination of treatment and services of agency clients.
  • Develops a system of tracking agency client statuses from referral to successfully securing intake appointment.
  • Develops and implements a system of follow up for clients who, after treatment has been established, require reengagement.
  • Conducts initial phone screenings and assessments to assess appropriateness and eligibility for services. Describes programs and services and discusses out-of-pocket expenses that may be applicable. Guide clients from their first screening call through the intake appointment.
  • Conducts agency intake assessments upon request of management.
  • Coordinates with agency staff and makes referrals to agency groups and programs. Develops a system of follow up to make sure clients successfully engage in the appropriate programs.
  • Make referrals to other community resources or agencies for services not provided by AZCFC, i.e., medication services.
  • Obtains initial authorization for services for clients who require authorizations.
  • Has extensive knowledge of and adheres to state licensure requirements regarding intake and admission to services.
  • Assists the agency Director in marketing agency programs and maintains positive relationships with referral sources to maintain a continual flow of referrals.
  • Assists the agency Director in coordinating meetings and presentations with community referral sources to explain our program and generate interest.
  • Outreach referring agencies successful scheduling of intake appointments and completion of intake appointments.

Qualifications:

  • Bachelor’s Degree or higher in a human service related field.
  • One or more years experience working in the behavioral health or human services field.
  • Basic computer literacy required, including data entry, web navigation, typing and text editing, and the ability to learn operations in new software systems.
  • Proficiency with Windows operating system, Microsoft Word, Excel, and Outlook.
  • Valid Level One Fingerprint Clearance Card or the ability to obtain one.
  • Is able to meet the demands of the position by consistent attendance and high quality work product.

Work Schedule: Mon-Friday: 10am-6pm.
