AuthoraCare Collective is currently seeking an RN On-call Integrated Health Services and Home Health position supporting our High Point, Kernersville and Winston-Salem service areas. This position is a Full-time (30 hours) Monday - Friday 4:00pm - 10:00pm.Must be able to travel to, enter, and function in the homes of any patient/caregiver within the service area.

Triages incoming calls for patients on Integrated Health Services, and Home Health. Assess and identify patient problems and respond appropriately with clinical intervention. Complete in-home clinical visits as applicable to assess and treat patient. Coordinate with appropriate resources to meet patient’s after-hours needs. Work closely with members of the care teams to ensure quality communication and patient care. Schedule and add appropriate documentation to EMR with each patient interaction including obtaining physician orders.

Complete assigned EMR workflow. Establish and maintain therapeutic relationship with patient, including providing disease education, medication management, and health recovery. Will send communication to appropriate discipline to ensure continuity of care.
