AuthoraCare Collective is currently seeking an RN On-call Home Health / Integrated Health Services position supporting our High Point, Kernersville and Winston-Salem service areas. This position is Full-time (30 hours) Saturday & Sunday 8:00am-8:30pm (flexibility with the remaining 6 hours in the evening(s).Must be able to travel to, enter, and function in the homes of any patient/caregiver within the service area.

Triages incoming calls for patients on Integrated Health Services, and Home Health. Assess and identify patient problems and respond appropriately with clinical intervention. Complete in-home clinical visits as applicable to assess and treat patient. Coordinate with appropriate resources to meet patient’s after-hours needs. Work closely with members of the care teams to ensure quality communication and patient care. Schedule and add appropriate documentation to EMR with each patient interaction including obtaining physician orders.

Complete assigned EMR workflow. Establish and maintain therapeutic relationship with patient, including providing disease education, medication management, and health recovery. Will send communication to appropriate discipline to ensure continuity of care. Education and Experience:

• Graduate of an accredited School of Nursing.
• Must be currently licensed as an RN in North Carolina and in good standings with NCBON.
• One year of experience in an acute, hospice, home health setting, or long-term care setting. Two years of experience preferred.
• Ability to prioritize and clinically triage patients appropriately.
• Ability to act with integrity, professionalism, and confidentiality.
• Patient management skills needed include excellent patient assessment and clinical nursing skills, critical thinking, problem solving, effective interpersonal and communication skills necessary.
• Current CPR certification
• Must be able to travel to, enter, and function in the homes of any patient/caregiver within the service area.
• Other: Valid state-issued driver's license required. Must carry automobile liability insurance at limits required by agency. Must have own transportation.
