AuthoraCare Collective is currently seeking an RN Case Manager for ourIntegrated Health Services (Skilled home-based-care) department supporting our High Point, Kernersville and Winston-Salem service areas. This position is Full-Time: Monday - Friday 8:00am - 5:00pm.

Manage the overall Plan of Care for a case load of Integrated health patients in collaboration with other members of the interdisciplinary team (IDT). Assess and identify patient/caregiver problems, establish nursing goals, and interventions. Work with physician to establish medical Plan of Care for the patient. Establish and maintain therapeutic relationship with patient, including providing disease education, medication management, and health recovery.

Work closely with other members of the interdisciplinary team and communicate effectively both verbally and in writing with team members, patients, caregivers, physicians, pharmacists, and other health care professionals. Document the goals and outcomes of care.Assume on call responsibilities as scheduled.
