AuthoraCare Collective is currently seeking an RN for our After Hours Weekend Nights position (Home Care and Long Term Care) that will support both our Burlington and Greensboro service areas. This position is Part-Time; Saturday and Sunday 8:00pm - 8:00am.

**Training must be completed during the work week between the hours of 8am-5pm/flexible on days**

Job Summary:

Assess and identify patient/caregiver problems, establish nursing patient/caregiver care goals, and identify and implement interventions for all agency programs. Will document outcomes of care. Work with physician to establish and implement medical plan of care for the patient. Work closely with other members of the interdisciplinary group and communicate effectively both verbally and in writing with group members, patients, caregivers, physicians, pharmacists, and other health care professionals and community service organizations. Establish and maintain therapeutic relationship with patient/caregiver to include providing emotional support and teaching.
