AuthoraCare Collective is currently seeking an Integrated Health Scheduler/LPN PSC (Patient Service Coordinator) for ourIntegrated Health Services (Skilled home-based-care) department supporting our High Point, Kernersville and Winston-Salem service areas. This position is Full-Time: Monday - Friday 8:00am - 5:00pm.

The Integrated Health Scheduler/LPN works directly with Integrated Health clinical manager and director to provide high quality clinical support to the Integrated Health patients through scheduling patients, navigating the EMR systems, checking in patients via EMR, updating provider calendars and schedules, telephonic or telehealth support, verify continued care as necessary, and following up with patients as requested in a clinic/community-based setting.

Responsible for completion of intake and setting the patient up in EMR. LPN will identify incomplete or noncompliant referrals and work to resolve any missing information. Communicate effectively both verbally and in writing with the Integrated Health Team, and community physicians. Identify patient needs in the home or facility and coordinate accordingly. Document the outcome of care in the appropriate EMR system.
