Augusta Health is seeking a part time Physical Therapist to join the Augusta Health team that is making a difference in the lives of our patients in the Shenandoah Valley by providing quality in-home care.
The Physical Therapist for Home Health will assess and implement plan of care needs to patients in a home setting and provide treatments to patients and evaluates progress as well as assist in the instruction of additional physical therapy personnel, students, home health aides, and volunteers. The individual must demonstrate the ability to interact with team members for an interdisciplinary team approach. Patient care is provided across the life span; therefore, clinicians will interpret data and assess patient’s needs relative to age specificity standards.
