Client Care Needs: Client is a 71-year-old female who is present with a fractured hip due to a fall, Hypona, GI Bleed. Caregiver needed to assist client with all ADLS/IADLS, bathing, dressing, meal prep, light housekeeping, toileting, med reminder, changing the linen. Laundry, transportation as needed and companionship.
Client has 2 cats.

Client is alert and oriented

Client has the following DME: walker, shower chair, grab bars, cane, bedside commode.

Lives Alone Y/N: Yes
Pets: 2 cats
Smoking: NO
Driving Required: YES
Do they have a car for CG to use: Yes

Friday's, Saturday's, Sunday's (Overnight shift)
Time 8p-8a
Duties: Strict Supervision, Fall Precautions
Or
Saturday's and Sunday's (Day shift)
Time 8a-8p

Pay: $17
Qualifications/Required Documentation:

High school or equivalent (Preferred).
Caregiving: 1 year (Preferred).
US work authorization. (U.S. passport or U.S. passport card. Form I-551, Permanent Resident Card or Alien Registration Receipt Card (this is commonly called a Green Card.) (Preferred).
CNA or HHA (40 Hours) Certificate (Accepted).
OSHA (Required).
Domestic Violence (Required).
Self-Assistance with Medications (Required).
Alzheimer's (Required).
Valid Driver's License & Social Security Card (Required).
CPR/AED/First Aid Certification (Required).
Auto registration & proof of Auto Insurance (Preferred).
Liability Insurance (CM&F rate is $69.00/yearly) (Required).
CEU certificates: HIV/AIDs, HIPPA,
Void check or equivalent (for bi-weekly direct deposit).
Level 2 and local background check. (Preferred).
2 professional references. (Required).
