General Statement of Responsibilities

The Program Coordinator is responsible for the overall operations of the assigned residential program. The Program Coordinator operates under the supervision of the Director of Residential Services. The primary responsibility of the Program Coordinator is to ensure Attain, Inc.’s policies and procedures are consistently implemented and to support the achievement Attain, Inc.’s stated goals.

Requirements

Note: Successful applicants must submit: 1) Cover letter, 2) Resume, and 3) Salary History. Applicants that fail to submit all 3 will not be considered for this position.

Preferred: BA/BS degree in Psychology, Social Work, Human Services or related area of study and a minimum of 2 years’ experience working with individuals with intellectual disabilities or 2 years of college and 4 years of experience in working with individuals with intellectual disabilities

Minimum of 1-year experience supervising Direct Support Professionals working in a residential setting.

Current CPR certification.

Florida driver’s license in good standing and good driving record.

Satisfaction of Agency for Persons with Disabilities level 2 background screening requirements (local law enforcement check, fingerprinting, etc.).

Knowledge regarding governing rules and regulations, standards and policies pertaining to residential habilitation provided in licensed group homes.

Computer knowledge, specifically Microsoft Outlook, Word and Excel.

Specific Duties

Hiring, Training, Scheduling and Managing, Employee Performance

Ensure staff members providing residential habilitation meet the minimum initial and ongoing training requirements as required by Chapter 393, Rule 65G, the Medicaid Waiver Handbook, the Provider Agreement, and Attain, Inc. policy. Ensure employee staff training is tracked and recorded to document compliance with staff training requirements.

Provide ongoing feedback to the Direct Support Professionals using positive and corrective feedback.

Develop, coordinate, and monitor staffing schedules. Monitor staff utilization daily. Coordinate shift coverage for vacancies/shortages to assure minimum staffing patterns are maintained while ensuring support services are provided in the most efficient cost-effective manner.

Residential Habilitation and Behavior Analysis Services

Ensure residential habilitation services are provided in accordance to Chapter 393, Rule 65G, the Medicaid Waiver Handbook, APD the Provider Agreement and in accordance to applicable Attain, Inc. policies, to ensure consumers are safe, healthy, enjoy their lives, become as independent as possible and overcome their problem behavior. Ensure that all direct care staff adhere to The Bill of Rights for People with Developmental Disabilities. Ensure the consumers are free of abuse and neglect. Assist with the admission, transfer, and discharge planning of all individuals served at Attain, Inc.

Coordinate residential licensing for assigned programs. Ensure the community residential homes maintain proper licensing through the Agency for Persons with Disabilities and meet the requirements of Chapter 393, Rule 65G, the Medicaid Waiver Handbook, APD the Provider Agreement and in accordance to applicable Attain, Inc. policies.

Ensure that all housekeeping, maintenance, and repairs are completed in a timely manner to maintain clean, healthy, and safe environments.

Review all incident and shift reports, taking appropriate action as needed in accordance with Chapter 393, Rule 65G, the Medicaid Waiver Handbook, the Provider Agreement, and Attain, Inc. policy.

The Program Coordinator will be on-call to support the delivery of residential habilitation and behavior analysis services in accordance with Chapter 393, Rule 65G, the Medicaid Waiver Handbook, the Provider Agreement, and Attain, Inc. policy.

Ensure that a family home atmosphere is maintained in the home. A family home atmosphere is one that promotes positive interactions, fosters dignity, and which allows for risk taking while fostering growth and independence.

Support the delivery of applied behavior analysis in accordance to Chapter 393, Rule 65G-4, the Medicaid Waiver Handbook, the Provider Agreement, and Attain, Inc. policy.

Ensure that adequate transportation is available and provided to residents as needed. Ensure that all vehicle safety standards are followed and adhered to by direct care staff.

Ensure residential habilitation and transportation services are documented in accordance to Chapter 393, Rule 65G, the Medicaid Waiver Handbook, the Provider Agreement, and Attain, Inc. policy.

Ensure Attain, Inc. compliance with Florida Administrative Rule 65G-8 Reactive Strategies and applicable Attain, Inc. policies.

Coordinate the implementation of Attain, Inc.’s consumer grievance policy and procedure.

Meet with parents, guardians, Support Coordinators, Agency for Persons with Disabilities representatives, employers, physicians, staff members, neighbors and all other professionals as required to provide high quality residential services.

Quality Assurance

Audit data collection records routinely to ensure compliance within accordance to Chapter 393, Florida Administrative Rule 65G, the Medicaid Waiver Handbook, the Provider Agreement, and Attain, Inc. policy.

Audit all required residential, consumer files, financial records, and service documentation and records routinely to ensure compliance in accordance to Chapter 393, Rule 65G, the Medicaid Waiver Handbook, the Provider Agreement, and Attain, Inc. policy.

Actively participate with Attain, Inc. quality assurance initiatives.

Financial

Assist with the coordination and auditing of consumer and agency petty cash and banking procedures in accordance to Chapter 393, Rule 65G, the Medicaid Waiver Handbook, the Provider Agreement, and Attain, Inc. policy. Ensure purchases are made in accordance to the purchase order and charge card policies.

Coordinate the payroll in accordance with applicable payroll policies and procedures.

Assist in implementing and maintaining within approved home budgets (i.e., purchase groceries, maintain inventory of all needed supplies).

Medical

Ensure the health, safety and well-being of each consumer. Assist with the healthcare coordination and medical appointment coordination in accordance to Florida Administrative Rule 65, and Attain, Inc. policy.

Ensure medication is stored, administered and documented in accordance to administration Florida Administrative Rule 65G-7. (Refer to the Medication Administration Policy and the Medication Administration Rule 65G-7.

Qualities

Ability to work effectively with persons with intellectual disabilities, their families, professional staff, and other support staff.

Ability to communicate effectively, both orally and in writing.

Ability to provide effective performance feedback.

Ability to effectively work with other team members.

Ability to prioritize and manage multiple projects simultaneously.

Ability to design and implement performance management strategies to assess and promote service delivery quality and outcomes.

Ability to demonstrate mature, stable, value-based conduct.
