A reputable supportive housing organization based in NYC is currently seeking MSW level Social Workers for their social service team in Manhattan. This organization strives to provide supportive housing services to seniors and homeless or at-risk individuals in a compassionate manner, through employing a range of interdisciplinary professionals. You would be tasked with assisting and providing social services to resident's within the organization's programs.

The base pay range for this position is $28 to $30 per hour. The actual compensation offered to the successful candidate may vary from posted hiring range based on geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of the offer. Responsibilities:

• Assist residents in obtaining and maintaining appropriate medical and mental health services.
• Assist residents in obtaining and maintaining entitlements.
• Assist residents in obtaining and maintaining home care.
• Provide care coordination for residents, as needed, between community service providers.
• Assist residents in establishing and/or maintaining family and social relationships.
• Through in-house and community referrals, assist residents in participating in socialization activities.
• Assist residents with budgeting, income maintenance etc. to ensure on-time payment of rent.
• Assist residents with money management as needed.
• Provide individual counseling as needed.
• Provide crisis intervention services as needed.
• Provision of group work services for residents, including recreational, educational and/or clinical groupwork.
• Complete non-reportable and reportable incident reports as assigned.
• Maintain thorough case records, including case notes, support plans and psychosocial histories .

Qualifications:

• MSW preferred, or MA/MS in other human services field with two years of social work experience
• Previous experience with supporting older adults and people with mental illness and/or substance abuse issues in a social service capacity
