- Collaborate with IDT Team to create care plans to meet the needs of individual patients and families.
- Communicate with patients and family as well as Skilled Nursing Facility Staff, Hospital Staff and other patient providers and caregivers to provide the best care possible.

- Document timely and accurately
- Ability to think creatively and work with physicians and family members to treat patients the most effective way possible.
