Atlantic Shores Rehabilitation & Nursing Center in beautiful Millsboro, DE is hiring for a Registered Nurse ADD-ON Coordinator Position.

Job Summary: In conjunction with the Rehabilitation Department; assesses residents for restorative needs. Develops individualized programs and plans of care based on the resident’s needs and abilities. Supervises Restorative Nurses Aides to ensure proper implementation of the restorative program, revises the plan as necessary and makes appropriate referrals to the rehab department.

Job Description:

• Collaborates with the rehabilitation department, nursing department and administration in implementation of the restorative program.
• Assesses resident’s abilities and determines needs related to maintenance and restoration of the resident’s highest practicable level of physical and psychological function.
• Develops individualized restorative care plans in conjunction with appropriate interdisciplinary team members.
• Reviews, revises, and documents outcomes of the restorative care monthly on the restorative nurse’s aide weekly documentation sheets.
• Education of direct care and nursing staff as needed in relation to the Restorative Nursing Program, Medicaid Reimbursement and Functional Care Summary documentation guidelines.
