Registered Nurse Case Manager
Duties:
- Provide case management services to patients in SNF, ALF, PCH and Pt's home.
- Assess patient needs and develop individualized care plans
- Coordinate and collaborate with interdisciplinary healthcare team members
- Monitor patient progress and adjust care plans as necessary
- Conduct regular patient assessments and evaluations
- Educate patients and their families on disease management and options
- Facilitate communication between patients and healthcare providers
- Ensure compliance with HIPAA regulations and maintain confidentiality of patient information
- Assist with discharge planning and coordination of post-discharge care
Requirements:
- Valid RN license in GA
- Minimum of 2 years of experience in case management or related field
- Strong knowledge of medical terminology, physiology, and critical care practices
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Proficiency in electronic medical records systems
- Familiarity with hospice care principles and practices
- Understanding of HIPAA regulations and patient confidentiality guidelines
