RN CASE MANAGER

LITHONIA/CONYERS/COVINGTON AREAS - $10K Retention Bonus.

Does Patient Visits and Is responsible for coordinating the care delivered to a caseload of patients and ensures care delivered is within quality and utilization guidelines. Assesses, plans, implements, monitor’s and evaluates actions taken as part of a comprehensive plan of care to ensure outcomes of care meet or exceed standards.

MINIMUM JOB REQUIREMENTS

Education & Qualifications:

Education: Graduate of an accredited Diploma, Associate Degree, or Baccalaureate School of Nursing. Preference is given to individuals with Baccalaureate Degree or higher

Licensure/Certifications: Must have current licensure by the Georgia State Board of Nursing and valid CPR certification. Must possess a current valid Driver’s license.

Knowledge/Training: Knowledge of Medicare, Medicaid and other funding source regulations as applied to home health care services. Demonstrate knowledge and competence in advanced practice nursing skills, including the nursing process, and collaborating with interdisciplinary team. Demonstrate knowledge of home health proficiency in quality of clinical records, productivity, customer satisfaction surveys and leadership role with staff. Must be able to function independently and effectively in a home-based work environment. Requires proven interpersonal, supervisory and management skills with ability to communicate effectively. Demonstrates self-initiative, proven decision-making skills and time management skills. Ability to operate computer equipment, FAX and copier machines as well as maintaining communication with Company telecommunication systems.

Pay: $83,000.00 - $100,000.00 per year
