Athelas Institute is Hiring a RESIDENTIAL CASE MANAGER- (MON-FRI) 9AM-5PM Near Columbia, MD

Job Title: Residential Case Manager
Location: Columbia, MD
Department: Administration
Hours: Monday - Friday 9am-5pm
Reports To: Director of Residential Services
Essential Job Functions:
  • Provide specific case management services to individual in residential programs.
  • Coordinate with community resources needed (medical, psychological, vocational, recreational, and supported services) based on the assessed needs and goals of the individual.
  • Assist individual in becoming eligible for financial and other resources, which will enhance their independence and quality of life.
  • Completes Representative Payee Reports for Social Security benefits on an annual or as needed basis.
  • Monitors Social Security, Medicare and Medicaid benefits to ensure benefits stay on line and services are not disrupted.
  • Maintain individual books and records, according to DDA and OHCQ regulations.
  • Review individual services on a minimum quarterly basis or as needed.
  • Prepare social summaries, and other necessary paperwork in conjunction with the annual PCP and in accordance with current DDA regulations.
  • Assist families, staff, and other in understanding the changing needs of the individual. Acts as an advocate for the individual and safeguards their individual rights.
  • Intervenes in daily crisis situations to assist individual in working through time-limited problems; as well as ensuring the most appropriate services are received.
  • Assist with coordination of transportation for individual between programs.
  • Attend regular meetings, including Core Review, Annual PCP’s, Case Management, and committee meetings.
  • Other duties may be assigned by the Executive Director or her designee.
Relationships:
Internal: Residential Department
External: Parents/Caregivers and Consultants
Education: 
Bachelors Degree in Behavioral Science, Social Work, Rehabilitation, Counseling, or related field.
Training and Experience: 
Experience working with Individuals with developmental and Intellectual Disabilities. Knowledge of community resources. Preferred DDA core competencies, Mandt Systems Training, CPR / First Aid, MTTP.
