Job Title/Position: Registered Nurse (RN)

Reports To: Clinical Director

JOB DESCRIPTION SUMMARY

The registered nurse plans, organizes and directs home care services and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

Patient Care

1. Completes an initial assessment of patient and family to determine home care needs.
Provides a complete physical assessment and history of current and previous illness(es).

2. Regularly re-evaluates patient nursing needs.

3. Initiates the plan of care and makes necessary revisions as patient status and needs change.

4. Uses health assessment data to determine nursing diagnosis.

5. Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process.

6. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician.

7. Counsels the patient and family in meeting nursing and related needs.

8. Provides health care instructions to the patient as appropriate per assessment and plan of care.

9. Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.

10. Acts as Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload.

Communication

1. Prepares clinical notes and updates the primary physician when necessary and at least every 60 days.

Job Title/Position: Registered Nurse (RN)

2. Communicates with the physician regarding the patient’s needs and reports any changes in the patient’s condition; obtains/receives physician’s orders as required.

3. Communicates with community health-related persons to coordinate the care plan.

Additional Duties

1. Participates in on-call duties as defined by the on-call policy.

2. Ensures that arrangements for equipment and other necessary items and services are available.

3. Instructs, supervises and evaluates home health aide care provided every two (2) weeks.

POSITION QUALIFICATIONS

1. Graduate of an accredited school of nursing. A minimum of One (1) year of experience in nursing practice as a Registered Nurse is required.

2. Current licensure in state and CPR certification.

3. Bachelor’s degree, with one (1) year of home health care experience preferred.

4. Management experience not required. Responsible for supervising home health aides.

5. Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.

6. Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.

WORKING ENVIRONMENT

Field experience may expose employee to temperature extremes, risk of physical injury by client, family, animal or others in the community, unsafe environment or exposure to infectious diseases and offensive odors.

1. Equipment and Supplies: Prudent and efficient use of routine office supplies and equipment. Prudent and efficient use of Electronic Devices used to document in the patient’s Electronic Medical Record. Prudent and efficient use of routine medical supplies, clinical bag contents, sphygmomanometer and stethoscope. All the preceding items should be protected from theft and weather at all times.

Job Title/Position: Registered Nurse (RN)

2. Knowledge and Competency: Must demonstrate basic knowledge of all procedures included in the Registered Nurse job. Must have the ability to follow instructions from the physician, Clinical Director and other professional staff and have the ability to communicate effectively with managers, clients, families and co-workers. Must have effective means of transportation, which will enable the employee to make visits to clients’ homes. Must be able to assess safety of home situations for self, clients, and other staff members, including physical and psychological dangers. Must have the ability to effectively cope with clients, families and all others with varying backgrounds, socioeconomic conditions, and value judgments. Skills include the ability to utilize a professional approach to problem solving in delivering patient care with considerations being given to the client’s past experiences, finances and other resources. Skills are needed to assess both physical and psychological needs of clients and institute the appropriate nursing intervention. Must possess the ability to handle medical and situational emergencies.

Maintains confidentiality of information relating to the client. Will discuss only those aspects necessary to the care and treatment of the client with those directly involved in the client’s care.

3. Physical Demands: Must be able to complete all physical demands of the job, which may include, but is not be limited to: the ability to lift, position and/or transfer adult patients; the ability to lift and carry supplies and equipment; ability to read normal typewritten print.
Have corrected vision and hearing within normal range and have manual dexterity with normal range of motion of all extremities. Ability to tolerate prolonged and considerable walking and/or standing. May be requested to work weekends, holidays, and occasional overtime. Must be able to work alone without usual support systems immediately available in acute care settings. Must have the ability to deal with abrupt schedule changes and to maintain objectivity in coping with the stress of working with acute, chronic and terminally ill clients.

4. Image Responsibility: Complies with the At Home Rehab dress code, projects a professional attitude toward self, clients, and other staff members, and follows department public relation policies regarding outside speaking and professional affiliations, clearing all such requests through At Home Rehab.

Pay: $65,000.00 - $82,000.00 per year
