Assessment and Care Planning
· Performs nursing assessments and reassessments as per policy.
· Ability to adequately assess and reassess pain.
· Continuously observes patient's behavior, mental status and activities.
· Formulates and implements patient plans of care in collaboration with multidisciplinary team.
· Revises plan of care as indicated by the patient's response to treatment and evaluates overall plan for effectiveness.
Patient Care
· Demonstrates the ability to perform crisis intervention and triage.
· Coordinates and supervises patient care as necessary.
· Communicates appropriately and clearly to the Nursing Director, co-workers, and Physicians.
· Collaborates with the interdisciplinary team to facilitate individualized, optimal care for patients.
· Treats patients with respect and dignity; provides emotional support.
· Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
· Supports and maintains a culture of safety and quality.
· Provides education to the patient concerning plans of care, compliance with the plan of care, medications and their side effects.
· Functions as a patient advocate at all times.
· Demonstrates ability to perform treatments and provide services to level of licensure.
· Obtain health histories and perform comprehensive physical examinations, including psychosocial, functional, and development assesments
· Order and interpret lab results and other diagnostic studies
· Develop therapeutic plan of care
· Evaluate patients response to plan of care and modify as needed
· Provide patient/family counseling and education
Medication Management and Administration
· Knowledgeable of medications, including psychotropic drugs, and their correct administration based on the age of the patient and their clinical condition.
· Follows the six (6) medication rights and reduces the potential for medication errors.
