What will you do?  As a Lead Therapist at Aspire HealthPartners, your job purpose will be to provide a variety of essential functions inthe provision of services to program participants.

These functionsinclude:

1.Providing therapeutic interventions inindividual and/or group settings to participants2.Creating, updating and monitoring progress onparticipant treatment plans3.Completing program specific assessments4.Documenting client treatment details in theElectronic Health Record5.Providing ongoing communication to the programteam regarding changes, issues or needs of participants or of the program ingeneral6.Assisting the program team as directed withthe operation of the program7.Providing appropriate discharge planning andreferral8.Maintaining compliance by completing necessaryand ongoing training and engaging in all staff meetings as directed9.Other duties as assigned

Qualifications

• Master's Degreein social services
• 1 year experienceworking with target population preferred
• Pass a Level IIBackground clearance
