Job Purpose : As the SSVF Coordinator , your job purpose will be to perform the functions required to ensure program outcomes are attained through the implementation and management of effective and efficient systems for the Supportive Services for Veteran Families (SSVF) Federal Contract CFR : : 38 CFR Part 62.

Job Functions

  • Responsible for the daily organization and coordination of the various administrative and support functions that promote smooth operations and service delivery in a manner consistent with best practices and excellent customer service.
  • In conjunction with leadership, responsible for developing, implementing, and maintaining policies and procedures and ensuring team professional development activities and documentation are carried out.
  • Oversees recruitment and onboarding of new team members to ensure the maintenance of a productive team while promoting excellent customer service to our patients, community, and colleagues.
  • Meet all required goals including timely billing and reimbursement of the contract.
  • The contract aims to design, plan, and work closely with Homeless Services Network to create seamless processes and develop tools to assist program staff deliver supportive case management housing services to promote equitable housing stability services to low-income Veteran Families who reside in or are transitioning to permanent housing.
  • Gather and upload required program documentation to follow the Homeless Management Information System (HMIS) and Client Track EHR data requirements.
  • Perform day-to-day operations and provide Supervision to the SSVF team.
  • Plan and implement a Temporary Financial Assistance (TFA) process for prompt check request, pick up, and delivery to landlords within specified lease agreements.
  • Review all program documentation to meet program requirements.
  • Most importantly, all functions must be completed while maintaining high standards of ethical and professional conduct while adhering to agency policies and procedures.

Position qualifications : Ability to receive and maintain a(n)

  • Requires a minimum of 5 years’ experience in case management with a strong knowledge base in Veteran services, behavioral sciences, and supervisory management techniques preferred.
  • Understands contracts, HIPPA, and governmental and professional standards related to behavioral health provided by the program.
  • Requires the ability to read, analyze, and interpret written information including regulations and effectively communicate verbally and in writing where information includes precise data and terminology, possibly of a confidential nature.
  • Requires strong computer and electronic health system skills.
  • Requires a valid State of Florida Driver’s license.
