Job Overview
Responsibilities
- Conduct telehealth and in-person sessions.
- Perform assessments (including vitals, sleep and nutrition)
- Manage and administer nursing care according to individualized care plans.
- Supervise Registered Nurses (RN’s) and Certified Nursing Assistants (CNAs) and collaborate with Care Teams and other healthcare professionals.
- Manage administrative tasks related to patient care, prescriptions, and lab tests.
- Ensure compliance with laws, regulations, and guidelines relevant to long-term care.
- Demonstrate effective interpersonal skills and work collaboratively within an interdisciplinary care team.
Qualifications
- Current, active RN license in the state of Utah.
- Clinical assessment skills at a level necessary for the job.
- Ability to make independent decisions when required.
- Knowledge of nursing practices, medical procedures, and relevant regulations.
- Effective interpersonal skills and collaboration within a care team.
- Positive attitude toward patient care, particularly in mental health and substance use disorders.
Required Experience
- Recent clinical experience, education, or specialty skills specific to geriatrics.
- At least one year of experience in Long Term Care preferred.
