Assessment Coordinator
The Assessment Coordinator is responsible for coordinating and completing all individualized assessments for youth referred to and enrolled in Wraparound services at Ascent's Baton Rouge location, serving W. Feliciana, E. Feliciana, Pointe Coupee, E. Baton Rouge, W. Baton Rouge, Iberville, and Ascension parishes. This position will ensure that all assessments are submitted to the Statewide Management Organization (SMO). The Clinical Director supervises this position.

Ascent Health, Inc. offers wraparound services for youth who are at risk of being placed out-of-home or in alternative school. The wraparound process is an intensive, individualized care management process for youths ages 5-20 with serious or complex needs. Research shows that the wraparound process does more for children than basic child welfare services alone.
