We are currently seeking an experienced and compassionate Home Health Registered Nurse, RN to join a prominent Home Health agency near Centerville, TN!

As a Home Health Registered Nurse, RN you will be responsible for providing exceptional care to multiple patients per day in the comfort of their own home! If you have a minimum of 1 year of recent Home Health experience visiting multiple patients per day in a home setting and you are dedicated to delivering compassionate care, we encourage you to apply for this lucrative position.

Responsibilities of Home Health Registered Nurse, RN:

• Conduct comprehensive assessments of home health and/or hospice patients in both stable and emergent situations, utilizing documentation, clinical records, case conferences, and evaluations to identify their physical, psychosocial, and environmental needs.
• Develop, implement, and evaluate individualized Plans of Care (POCs) to ensure high-quality and consistent care delivery, documenting all interventions and adjustments made.
• Gather data on various factors, including physical, psychological, social, spiritual, and cultural influences on patient and family health statuses, and integrate this information into the POC.
• Ensure continuity of care by maintaining accurate and up-to-date patient records, documenting all interactions, assessments, interventions, and changes as they occur, to uphold standards of quality patient care.

Compensation for Home Health Registered Nurse, RN:

• $42-$48/HR ($1680 -$1920/Wk) Mileage Reimbursement (paid out weekly via direct deposit every Friday morning)
• Mileage ($0.41/per mile from first patient to last patient per day)
• Additional benefits provided.

Schedule:

• Monday - Friday 8 HR Days
• On Call | Weekends: Not Required
• Productivity Requirements: 30 Units per week
• 5-6 Visits Per Day on Avg
• Length: 13 Week Contract (With Multiple Extensions Possible)
• Territory: Jefferson County & Small part of Shelby County

Requirements for Home Health Registered Nurse, RN Position:

• Active Registered Nurse, RN license in Tennessee or Compact License
• Minimum of 1-2 years of recent experience in Home Health
• Home Care Home Base (HCHB) EMR Experience Required
• OASIS Experience -Required
• Active Motor Vehicle Insurance - Required
• Active Driver's License - Required
