Arthur Center is Hiring a Medical Case Manager Near Mexico, MO

Overall Responsibility
The Medical Case Manager provides team-based care to patients in a patient-centered approach assuring access to supports, assure continuity of care, health promotion, population management to patients with chronic health conditions or patients at risk of health disparities.

Key Tasks and Responsibilities

  • Manages data entry and tracking to support a disease management approach for chronic conditions
  • Provides case management support for wellness support and basic needs for patients
  • Works with clinicians to monitor health and follow-up with patients in development of treatment plans and care coordination
  • Educates and encourages patients in the area of daily living skills
  • Facilitates progress toward recovery through resource development and utilization
  • Advocates effectively for patients
  • Provides timely, accurate and quality documentation.
  • Obtains healthcare information from other health care providers
  • Schedules appointments with other health care providers
  • Able to prioritize workload.
  • Works as part of an integrated team.
  • Provides access to vocational, financial, medical, housing, utilities, food, transportation and other social drivers of health.

Skills and Attributes

  • Possesses empathy skills as evidenced by good eye contact, listening skills and ability to develop rapport with a diverse range of consumers.
  • Has an array of multiple interventions they can draw from in working with consumers.
  • Has a basic understanding of medical terminology, medications and procedures
  • Familiar with electronic health records
  • Able to be flexible, non-judgmental, open-minded and accepting while still being able to maintain healthy professional boundaries.
  • Has a willingness to learn, grow and change.
  • Possesses a solid working knowledge of community resources and the ability to find and develop new resources.
  • Can work effectively with a diverse population of individuals both culturally and from a disability perspective
  • Is self-directed and able to work independently using sound judgment.
  • Strong team player
  • Has a solid knowledge of the provision of integrated care
  • Has a working knowledge of dual diagnosis client needs to include MI/MRDD, and MI/ADA.
  • Must have a safe driving record

Physical Requirements

  • Must be able to work in a general office setting routinely lifting less than 25 lbs.
  • Able to provide physical assistance to persons with disabilities in transfers in/out of vehicles
  • Able to sit for extended periods
  • Able to drive and provide transportation in the community

Degree Requirements
Meets at least one of the following:

1. Has a bachelor's degree in social work, psychology, nursing, criminal justice, recreational therapy, human development and family studies, counseling, child development, gerontology, sociology, human services, behavioral science and rehabilitation counseling or is a licensed practical nurse (LPN) with 3 years of experience in a primary care setting.

Licensure Requirements
Class E Driver’s License

Certifications: CPR/First Aid, Nonviolent Crisis Intervention, Level 1 Medication Aid.

Experience Requirements
One (1) year experience in a health care setting. Preference for those with FQHC or patient-centered medical home experience.

Job Type: Full-time

Pay: $21.50 - $27.00 per hour

Expected hours: 40 per week
