Position: RN Case Manager
Location: Boston, MA
Schedule: 8-hour days, Monday to Friday, 7:30 am to 5:00 pm; flexibility to move to 4x10s after the first week of orientation
Requirements:
- Active RN license in Massachusetts
- Minimum of 2 years of experience in case management
- Strong communication and organizational skills
- Ability to work independently and collaboratively within a multidisciplinary team
- Experience in travel nursing preferred
Roles & Responsibilities:
- Conduct comprehensive assessments of patients' healthcare needs
- Develop individualized care plans in collaboration with the interdisciplinary team
- Coordinate and facilitate patient care transitions, including discharge planning and referrals to community resources
- Monitor patient progress and adjust care plans as needed
- Provide education and support to patients and their families regarding their healthcare needs and treatment plans
- Collaborate with physicians, nurses, social workers, and other healthcare professionals to ensure continuity of care
- Document patient assessments, care plans, and interventions accurately and timely
- Participate in departmental meetings, quality improvement initiatives, and continuing education activities
- Adhere to all regulatory and compliance standards, including HIPAA and CMS guidelines
