$500.00-$800.00 Sign On Bonus Paid Out as $200 each month of the first 120 Days for full/part time and after working 300 hrs for on-call receive $500 (Must have perfect attendance and no disciplinary action on file)

Arizona Autism is looking for an experienced Direct Care Worker looking to make a difference in the lives of our Adult members who are co-ed living in a family style setting. We are looking for individuals who truly have a caring heart and a passion for our clients.

Responsibilities of the Residential Group Home Direct Care Worker include, but are not limited to:

ISP:

• Carry out and coordinate with GH Manager and GH Director for the implementation of the Individual Service Plans (ISP)
• Carry out and document submitted ISP and BTP goals.
• Monitor and advocate for new goals and past goals for the member to live an auspicious lifestyle.

Support:

• Facilitate professional interactions with both internal and external stakeholders.
• Provide support to individuals receiving services including assistance with daily activities such as meal preparation and planning; medication administration; and personal care.
• Properly track low volume medications for each consumer present in the facility.
• Facilitate community involvement by accompanying individuals on outings in the community or providing transportation to work, medical appointments (as needed), or other activities.

Financial:

• Provide receipts and financial documentation to GH Manager for consumer outings.
