We are seeking a Registered Nurse to join our home health team. The ideal candidate will have a passion for providing quality care to patients in their homes and will be responsible for assessing patient needs, developing and implementing individualized care plans, monitoring patient progress, and providing education and support to patients and their families. The successful candidate will also be responsible for documenting all care activities and maintaining accurate records.

Responsibilities:

• Provide patient assessments and develop individualized care plans.
• Provide direct nursing care to patients in their homes.
• Educate patients and families on disease processes, medications, and treatments.
• Monitor vital signs and administer medications as prescribed.
• Provide emotional support to patients and families.
• Maintain accurate records of patient care.

Job Types: Part-time, Per diem

Pay: $40.00 - $55.00 per hour
