A $6,000 Sign-on Bonus for Full-Time or $3,750 Sign-On Bonus for Part-Time is being offered with this position!

Provides assistance in daily living and personal care in patients' homes in accordance with an established care plan.

• Follows the assigned plan of care and notifies staff nurse of changing or unusual conditions. 
• Assists patients by providing personal services such as grooming, bathing and dressing. 
• Supports patients by providing housekeeping and laundry services; shopping for food and other household items; preparing and serving meals and snacks and assisting with eating; as defined in assigned plan of care. 
• Assists client with mobility/transfer, getting in and out of bed or chair and/or walking. 
• Obtains emergency assistance if required.
• Attends to the comfort and safety of the client during the caregivers' absence. 
• Participates in all scheduled in-service meetings. 
• Projects a professional image and attitude at all times. Wears company issued ID badge.
• Properly utilize applications as required such as Paylocity, Authenticare, MatrixCare, HHAExchange, and Carebridge. 
• Other duties as assigned.

Requirements

• Minimum of a High School Diploma or equivalent.
• Must possess a valid driver’s license.
• Reliable transportation and proof of state required vehicle insurance.
• Current negative TB skin test or state health card.
• Completion of 75 hour Home Health Aide training class.
• Certified in CPR/First Aid.
