**Position Details**
Must have AZ license and Arizona DEA!! Must have proof of the following immunizations: MMR, Varicella, Tdap, TB Skin Test as well as clean background
24 hour call coverage requested with 7-9 in-house hours per day

Phone Consults: 2-3
Call Ratio: 9 hours on site, 15 hours unrestricted
