We are seeking to hire an experienced Social Services Coordinator to join our team.
Our goal for Arcadia Medical of Talbot is to provide our residents with quality care. We desire individuals who have a passion for helping people, who thrive our team environment, and who want to make a difference all while advancing their skills.
Responsibilities:
- Complete initial resident assessments
- Implement care plans
- Assist with discharge planning
Document patient progress
Coordinate family meetings
Ensure regulatory compliance
Complete MDS sections as assigned, ensure all areas are addressed in thecare plan as necessary assigned, ensure all areas are addressed in the care plan as necessary

Requirements:
- A bachelor's degree in social work or a closely related field preferred
- A master's degree a plus
- State licensure preferred (CSW, ACSW, LSW, LCSW)
- Interpersonal, critical thinking, and organizational skills
