Summary of Job Functions:

The Case Manager oversees the delivery of services and supports to individuals in accordance with each individual’s identified goals as well as The Arc’s Mission Statement and Guiding Values. The Case Manager will participate in the coordination of care, individual support, and caseload management and assist individuals in achieving their goals in their residence and community.

The Case Manager is accountable to the person receiving support, their team, and the agency.

Essential Job Functions:  

  • Works in collaboration with the resident, staff, and resident support to create, achieve, and update treatment plans as needed.
  • Promotes and implements support in accordance with each individual’s Treatment Plan as well as The Arc’s Mission Statement and Value Guiding Service Principles.
  • Transports individuals to offsite appointments, groups, and services as directed by their treatment plan (ex. Medical appointments, community resources, support groups working on treatment plan goals, etc.). 
  • Provides linkage to appropriate community services and assists individuals in applying for and appropriately using community services.
  • Supports residents and team in the day-to-day functions of a recovery-oriented community and maintains a safe environment for the individual by following safety rules, policies, procedures and instructions.
  • Works in collaboration with The Arc staff and community resources to assure referral lists and opportunities are up to date. 
  • Adheres to all policies and procedures as outlined in the Employee Handbook and other departmental procedure manuals.
  • Facilitates groups and provides one on one support as needed to encourage and develop healthy and supportive coping strategies. 
  • Assures completion and processing of documentation required in the required timeframe for everyone supported (ex. EVS checks, service notes, etc.).
  • Actively participates in all required training, in-services, and staff meetings.
  • Facilitate team meetings.
  • Works closely with AMHR team and other interdisciplinary teams as apply (ex. guardians, family, care coordinators, and probation officer) to assess progress, concerns, and needs of the individual supported.
  • Understands and follows procedures for being a mandated reporter.
  • Ability to gather all information necessary to make appropriate decisions independently.
  • Ability to assist individuals either one on one or in small groups toward making progress on their goals. 
  • Have use of a personal vehicle with proper insurance during work hours. 
  • Responsible for sharing in an on-call system to provide support to staff and individuals receiving services.
  • Works in collaboration with AMHR team members with scheduling, shadowing and training with assigned individuals.
  • Perform other work-related duties as assigned. 

Education and Experience: 

  • High School Diploma and/or GED. One (1) year of experience working with or having direct life experience with individuals who experience mental illness. 
  • Preferred, associate degree in Social Work, Human Services, or a related field. Three (3) years of experience working with or having direct life experience with people who experience mental illness.
  • Basic computer use and experience with Microsoft Office.

Required Licenses, Clearances & Training:

  • Background checks as defined by licensing agencies
  • Current TB test/clearance
  • Valid driver’s license and proof of insurance 
  • Clean driving record 
  • Pass mandatory agency training (i.e., CPR, Mandt, Programs)
  • Completed all departmental training

Equipment Used:

  • Personal Vehicle 
  • Routine use of standard office equipment such as computers, phones, photocopiers, filing cabinets, and other office-related devices.

Working Conditions: 

  • Required to drive in the community with individuals served
  • Required to perform duties in the community 

Physical Demands: 

  • Sitting in a normal seated position for extended periods of time
  • Standing and moving for extended periods of time
  • Ability to exert 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or 10 to 20 pounds of force constantly to move objects. Physical demand requirements are in excess of those for light work
  • Communication skills using the English spoken word
  • Ability to see within normal parameters
  • Ability to hear within normal range
