We provide unique opportunities, create new living environments, and expand programs that fully encompass the needs of families. As a Mental Health Case Manager at Hope, you will play a crucial role in providing essential services to support recipients and their families. Your responsibilities will include case management, crisis intervention, advocacy, and coordination of community-based services. By facilitating ongoing collaboration between patients, families, and service providers, you will help achieve positive program outcomes and promote recovery.

Hope's Mental Health Case Manager provides the following services:

  • Community Recovery Support Services (CRSS)
  • Case Management (CM)
  • Intensive Case Management (ICM)
  • Associate degree in Health Science, Social Science, Human Services, Psychology, or related field; or,
  • Two years of experience working directly with individuals who experience a mental illness or behavioral health challenges; or,
  • One year of experience providing direct case management services including extensive coordination of services, collaborating with extra-agency treatment team members, and provision of direct treatment services; or,
  • Equivalent combination of related education and/or work experience.
