About the Role
The Transfer Coordinator is expected to perform a variety of duties, working closely with the Astrana Health On-Call Hospitalist in order to provide the highest quality service to our members.
On Call Coordinator will provide and coordinate information with outside providers, Emergency Rooms, PCP’s and offices, skilled nursing facilities, urgent care centers, outpatient surgical centers, home health, hospice, ambulance, etc. What You'll Do :
Each hospital ER department follows different protocols when calling ambulance transport. Their protocol has to be followed
