Position Title: Registered Nurse – TCU
Pay Rate: $50-$55/Hourly
Work Location: Torrance, CA
Shift: Night 5x8's 11pm-7am
Hours/day: 8Hours
Contract Duration: 13 Weeks

Licenses Requirement: CA license required
Certification Requirement: BLS
Min 2 years of experience within specialty
Prefer experienced traveler due to high volume /pace.

Job Title: Registered Nurse (RN) - Transitional Care Unit (TCU)

Job Overview: We are seeking a compassionate and experienced Registered Nurse (RN) to join our team on the Transitional Care Unit (TCU). The RN will be responsible for providing high-quality nursing care to patients transitioning from acute care to home or long-term care settings. The ideal candidate will possess strong clinical skills, excellent communication abilities, and a dedication to patient-centered care.

Responsibilities:

• Assess patients' health status and develop individualized care plans in collaboration with the interdisciplinary team.
• Monitor and administer medications as prescribed, ensuring accuracy and adherence to established protocols.
• Provide direct patient care, including wound care, catheter care, medication administration, and other treatments as necessary.
• Monitor patients' vital signs, assess changes in health status, and intervene promptly to address any concerns.
• Educate patients and their families about their conditions, medications, and self-care techniques to promote optimal health outcomes.
• Collaborate with physicians, therapists, and other healthcare professionals to coordinate and implement comprehensive care plans.
• Document patient assessments, interventions, and outcomes accurately and timely in electronic medical records.
• Ensure compliance with regulatory standards, policies, and procedures to maintain a safe and therapeutic environment for patients and staff.
• Participate in quality improvement initiatives, including data collection, analysis, and implementation of evidence-based practices.
• Provide emotional support and comfort to patients and their families during times of transition and adjustment.
