Job Overview:
We are seeking a compassionate and dedicated Medical Social Worker to join our team at American Home Care. As a Medical Social Worker, you will play a vital role in providing support and assistance to patients and their families after their hospital discharge in the home care setting.
Responsibilities:
- Conduct comprehensive assessments of patients' social, emotional, and environmental needs
- Collaborate with healthcare professionals to develop individualized care plans
- Provide counseling and support to patients and their families regarding medical conditions, treatment options, and available resources
- Assist patients in accessing community resources such as financial assistance programs, support groups, and home health services
- Advocate for patients' rights and ensure that their needs are met within the healthcare system
- Document all interactions and interventions accurately and in a timely manner
- Stay up-to-date with current research and best practices in medical social work
Skills:
- Strong knowledge of documentation practices
- Familiarity with home care setting
- Excellent communication skills, both verbal and written
- Strong organizational skills with the ability to prioritize tasks effectively
Qualifications:
- Bachelor's or Master's degree in Social Work or related field
- Valid NJ state licensure or certification as a Medical Social Worker
- Previous experience in a healthcare setting preferred
