Qualifications:

Required:

• Master s Degree in Social Work
• Excellent verbal and written communication skills
• Capable of working on multiple tasks simultaneously
• Expert critical thinking and problem-solving skills
• Self-motivation
• Can break down barriers
• Good organization and time management skills

Certifications and Licenses Required:

• NJ Licensed Social Worker

Essential Functions:

• Conduct biopsychosocial for all patients, engage families and/or community support services to assess patients' psychiatric, social, environmental and financial status and meet identified needs
• Develop treatment plans with patients and the interdisciplinary team
• Participate in interdisciplinary treatment planning rounds to assess discharge planning needs, treatment modifications, family involvement, and communicate ongoing interventions
• Educate and engage patients and/or families about treatment, discharge planning, patient needs, as well as availability and eligibility criteria for programs/services benefits to meet those needs
• Creatively negotiate, coordinate, and advocate community-based services to assure the highest level of needed supports, continuation of care and wrap around services
• Document in the Medical Record all assessments, reassessments and interventions to enhance multidisciplinary communications, as well as, to meet all regulatory and reimbursement standards
