We seek a passionate, skilled Case Coordinator to strategically support our Community Integration model and ensure veterans, their families, and caregivers are aware of the resources within their community and connected to those resources. Our Case Coordinator has two or more years of demonstrated success in building and maintaining relationships with veterans, their families and caregivers engaged through outreach efforts and who are connected to local resources. Our ideal candidate has strong interpersonal skills and a passion for the veteran population.

Location: Candidates must live in the Permian Basin region of Texas.

Key Duties and Responsibilities:

• Work directly with warriors to assist them in setting and reaching goals, accessing resources, and solving their challenges.

• Serve as the primary point of implementation for the program services received by warriors and their family members. This person works directly with warriors and their family members in achieving their goals.

• Conduct intake and assessments with individual warriors to discover needs/wants and develop a holistic warrior care plan that will guide the delivery of services and resources.

• Advocate for warriors by assisting in the expansion of community resources and maintaining cooperative relationships with community groups, partners, and resource agencies.

• Develop and maintain a system of referrals across a broad spectrum of agencies, including medical, nonprofit, and government agencies.

• Ensure that direct-service partners provide comprehensive warrior care to veterans receiving assistance according to their holistic warrior care plans.

Required Knowledge and Skills:

• Minimum 2 year experience in a social service role; or equivalent skills.

• Bachelor's degree in social work, social science, or related field; or equivalent experience.

• Experience in crisis management as well as comfort regularly discussing the topic of suicide and suicide prevention.

• Proficiency in using Microsoft Office and CMS (Salesforce) software

• Preference for previous military/veteran or spouse/family member of military/veteran.

• Must be currently legally eligible for employment in the United States.

Working Conditions:

• Regular working conditions are in a home office.

• Working for an extended period on a computer and/or telephone is required.

• Up to 20% of travel in the designated community

• Access to reliable personal transportation

• Periodically work evenings and on weekends to travel, attend meetings, community forums, conferences
